Almagel a - eliminating the clinical manifestations of gastritis. Almagel - instructions for the use of suspensions and tablets, how to take adults, analogues How to use almagel before or after meals

Almagel is rightfully considered one of the most popular antacid drugs, the indications for the use of which are not limited to the removal of heartburn. It is widely used for gastritis, ulcers, duodenitis, esophagitis and other acid-dependent diseases. It has good enveloping and adsorbing properties. Yellow Almagel, moreover, effectively removes pain in the epigastric region with digestive disorders. Available as a white suspension.

Sometimes the name of the drug is written with a soft sign after the first syllable "Almagel". But on the packaging of the product sold in the territory of the Russian Federation, there is no soft sign. It can be concluded that the spelling "Almagel" is more correct.

Composition and pharmacological action

At the moment, there are three varieties of Almagel on the market, they have common composition and indications, but each has its own peculiarity. The main active ingredients in the line are (dosage based on a measuring spoon of 5 ml, included):

  • Algeldrate in gel form, corresponds to 220 mg of aluminum oxide.
  • Magnesium hydroxide as a paste, corresponds to 75 mg of magnesium oxide.

Belong to non-absorbable antacids. The main advantage of this group is the duration of action, the absence of the phenomenon of "acid rebound" and a number of secondary positive effects.

  • The main property of all antacids is the ability to lower the level of acidity in the lumen of the stomach, due to the neutralization of hydrochloric acid.
  • Reduce the proteolytic activity of pepsin, an important component of gastric juice. Make the environment less hostile.
  • They have enveloping properties, while protecting the gastric mucosa.
  • Helps accelerate the healing of erosive and ulcerative lesions.
  • Bind bile acids and toxic substances.

Algeldrate has a fixing effect, like all aluminum compounds. And magnesium hydroxide is not only an antacid, but also an osmotic laxative (increases the water content in the feces and accelerates their transit through the intestines), thereby leveling each other.

Varieties

Almagel for gastritis is convenient to use due to its versatility. It is possible to choose exactly the composition that is best suited at the moment and to a specific ailment.

  1. Almagel - The classic representative of the line is available in the form of a suspension, in bottles with a green marking. Almagel for heartburn, if it is without complications, this type is prescribed. Unlike the rest, it does not have additional components, but is prescribed by doctors even more often, since their presence does not always lead to an increase in the desired effect. .
  2. Almagel Neo - An additional component of Simethicone. Belongs to the group of carminatives, defoamers. The chemical structure is an organosilicon compound. It reduces the surface tension of gas bubbles in the digestive tract, which stimulates their destruction and complicates their formation. The released gases are partially absorbed by the intestinal walls or excreted with peristalsis in a natural way. Simethicone itself is not absorbed, therefore it is considered safe, it is excreted unchanged.
  3. Almagel A - A feature is the presence of a local anesthetic Benzocaine (aka Anestezin). When it hits the mucous membrane, it blocks the conduction of nerve impulses, due to which pain and discomfort disappear almost immediately. Its disadvantage is a short-term effect, on average 2 hours. Oral absorption is minimal, but long-term carcinogenicity studies have not been conducted. In some cases, the analgesic effect will be a definite plus, but the list of side effects and contraindications is growing. Treatment longer than a month is not recommended. Available in yellow bottles.

Classic, Almagel Neo, Almagel A, what is the difference, it becomes clear after carefully examining the composition. The first Almagel for heartburn and other dyspeptic disorders will be the best choice. The second is more expedient to prescribe for bloating, flatulence and before various studies. The third is for pain in the stomach, with acute gastritis and other conditions.

Due to the obvious color differences between the packages, they are often called by color: Almagel green - classic. Almagel yellow - with anesthetic. Almagel red (orange) - with simethicone.

  1. The release form in the form of a gel allows the drug to be evenly distributed throughout the digestive tract, this provides reliable protection of the mucous membrane and the rapid onset of the desired effect.
  2. After taking, there is no sharp decrease in the level of acidity, this avoids problems with the digestion of food.
  3. When the acid is neutralized, carbon dioxide is not released, as a result of which there is no flatulence and, after a short time, the reverse reaction (as with absorbed antacids).
  4. With prolonged use of Almagel, you need to know that it is able to increase the excretion of phosphorus from the body, this can lead to deficiency. This fact should be taken into account when preparing food. For example, a large amount of it is found in nuts and legumes.
  5. Perhaps increased excretion of calcium ions in the urine, the appearance of edema. This should be taken into account for people with chronic kidney disorders.

Indications for use

  • Gastritis with high acidity.
  • Peptic ulcer of the stomach (and duodenal ulcer) during the period of periodic exacerbation.
  • Almagel relieves heartburn attacks well.
  • Esophagitis.
  • Duodenitis.
  • Enteritis.
  • Reflux esophagitis.
  • Food intoxications.
  • Bloating and flatulence.
  • Pain and discomfort caused by food, alcohol, drugs and other factors.

Almagel is used for prevention in the treatment of non-steroidal anti-inflammatory drugs, to reduce their ulcerogenic effect on the stomach.

Contraindications

  • Hypersensitivity to substances in the composition.
  • Severe hepatic pathology. Although the drug itself is excreted by the intestines, there is a risk of kidney damage due to increased excretion of calcium in the urine.
  • With caution during pregnancy.
  • Alzheimer's disease.
  • Predisposition to low levels of phosphate in the blood.
  • Fructose intolerance, due to the content of sorbitol.

Almagel is allowed for children from the age of ten. At an earlier age, the use is possible according to the strict indications of a doctor.

Write in the comments.

Do not hesitate to understand the problem that interests you together.

Mode of application

The dosage of Almagel varies according to the age and severity of the disease. Adults starting from 5 ml three times a day, up to 10 ml four times a day, if necessary, a single use of 15 ml of the drug is possible. For children 10-15 years old, the dosage is half that of an adult: 10-20 ml per day, divided into several times. For children under 10 years of age, the appointment is strictly under the supervision of a doctor.

Before use, the vial must be thoroughly shaken to give the suspension uniformity. It is not recommended to drink liquids in the first 30 minutes after ingestion. The last dose is taken at bedtime.

Analogues

There are preparations similar to Almagel, with the same composition as the classic form:

  • Maalox.
  • Maalox mini.
  • gastracid.
  • Agiflux.
  • Alumag.

Use during pregnancy

Doctors sometimes allow Almagel to be taken during pregnancy during heartburn attacks. With some conditions: not in the first trimester and lasting no more than three days. For an adult, the amount of aluminum that enters the body is extremely small, and severe side effects associated with this are rarely observed. For the fetus, penetrating the placental barrier, the dosage may be critical.

It is known that aluminum is a neurotoxic metal, accumulating in the bones, liver, brain, causing a number of complex pathologies. The reactions of the nervous system to intoxication are varied: the development of Alzheimer's disease, parkinsonian dementia. Promotes bone fragility, osteoporosis and rickets.

Self-administration of Almagel to pregnant women is contraindicated.

Conclusion

Preparations of the Almagel line, the indications for the use of which differ slightly depending on the variety, meet the modern requirements for antacids. Their use, according to the instructions, can be considered completely safe, and the effectiveness of the treatment of gastritis, heartburn, ulcers and other disorders has been proven by many clinical studies.

Almagel A- This is an antacid in the form of a suspension, is a local anesthetic.
Almagel A neutralizes free hydrochloric acid in the stomach, which leads to a decrease in the digestive activity of gastric juice. Does not cause secondary hypersecretion of gastric juice. It has a local anesthetic, adsorbing and enveloping effect, reduces the effect of damaging factors on the mucous membrane.
The therapeutic effect after taking the drug occurs after 3-5 minutes and lasts an average of 70 minutes.
Almagel A provides long-term local neutralization of continuously separated gastric juice and reduces the content of hydrochloric acid in it to the optimal limits for treatment. Aluminum hydroxide inhibits the secretion of pepsin, neutralizes hydrochloric acid, forming aluminum chloride, which in the alkaline environment of the intestine turns into alkaline aluminum salts. Magnesium hydroxide also neutralizes hydrochloric acid, turning into magnesium chloride. Thus, the effect of aluminum hydroxide, which causes constipation, is counteracted. Magnesium hydroxide and magnesium chloride are resorbed to a small extent and practically do not affect the concentration of magnesium ions in the blood.
Benzocaine has an effective and long-lasting local analgesic effect in severe pain syndrome.
Sorbitol, which is part of the preparation, promotes increased secretion of bile and exhibits a slight laxative effect, complementing the action of magnesium hydroxide.
Almagel A does not sharply increase the pH of the contents of the stomach, buffering its value from 4.0-4.5 to 3.5-3.8 (physiological value) between doses. The drug forms a protective layer, ensuring uniform distribution of active substances on the gastric mucosa and has a long-term local effect without the subsequent formation of carbon dioxide in the stomach, which, in turn, is the cause of flatulence, a feeling of heaviness in the epigastric region and a secondary increase in the secretion of hydrochloric acid.
According to the classification of Hodge and Sterner, the drug, when administered orally, belongs to slightly toxic agents and does not have embryotoxic, teratogenic and mutagenic effects. In some cases, increased tendon reflexes were noted in newborns whose mothers took the drug for a long time. In addition, in newborns there is a risk of developing hypermagnesemia, especially in a state of dehydration, therefore, prolonged use of the drug by pregnant women and newborns is not recommended.

Pharmacokinetics

Almagel A- non-absorbable drug. Subject to the correct dosing regimen and duration of treatment, it is practically not resorbed in the gastrointestinal tract and has a long-term uniform effect without disturbing the electrolyte balance and without creating a risk of alkalosis or other metabolic disorders. It does not irritate the urinary system and, with prolonged use, does not cause alkalosis and the formation of stones in the urinary tract.

Indications for use

Indications for the use of the drug Almagel A are: peptic ulcer of the stomach and duodenum in the acute phase; acute gastritis and chronic gastritis with increased and normal secretory function in the acute phase; duodenitis; enteritis; hiatal hernia; reflux esophagitis; functional bowel disorders, colitis; discomfort and pain in the epigastrium with errors in the diet, after drinking coffee, nicotine, alcohol; prophylactically in the treatment of corticosteroids and NSAIDs.
In the complex of therapeutic measures, Almagel A is prescribed to patients with diabetes.

Mode of application

suspension Almagel A take inside. As prescribed by the doctor, 1-3 dosing (tea) spoons, depending on the severity of the case, 3-4 times / day half an hour before meals and in the evening before bedtime.
In children, the drug is used strictly according to the doctor's prescription: children under 10 years old are prescribed 1/3 dose for adults, and children from 10 to 15 years old - 1/2 dose for adults.
In diseases accompanied by nausea, vomiting and abdominal pain, treatment begins with Almagel A, and after the disappearance of these symptoms, they switch to Almagel.
The vial should be shaken before taking.

Side effects

In some cases, when taking the drug Almagel A there may be a change in taste sensations, nausea, vomiting, stomach cramps, epigastric pain and constipation, which disappear after dose reduction. At high doses, it may cause drowsiness.
Long-term treatment with the use of high doses of the drug and food poor in phosphorus can lead in predisposed patients to the development of phosphorus deficiency in the body, increased resorption and excretion of calcium in the urine and the occurrence of osteomalacia. Therefore, with prolonged use of the drug, it is necessary to ensure sufficient intake of phosphorus with food.
In patients with chronic renal failure, in addition to osteomalacia, edema of the extremities, dementia and hypermagnesemia can be observed.

Contraindications

PHARMACHOLOGIC EFFECT

Pharmacodynamics
Almagel A is a balanced combination of algeldrate (aluminum hydroxide), magnesium hydroxide and benzocaine. It neutralizes free hydrochloric acid in the stomach, reduces the activity of pepsin, which leads to a decrease in the digestive activity of gastric juice. It has an enveloping, adsorbing effect. Protects the gastric mucosa by stimulating the synthesis of prostaglandins (cytoprotective action). This protects the mucous membrane from inflammatory and erosive-hemorrhagic lesions as a result of the use of irritating and ulcerogenic agents such as ethyl alcohol and non-steroidal anti-inflammatory drugs (eg, indomethacin, diclofenac, aspirin, acetylsalicylic acid, corticosteroid drugs). The therapeutic effect after taking the drug occurs after 3-5 minutes. The duration of action depends on the rate of gastric emptying. When taken on an empty stomach, the action lasts up to 60 minutes. When taken one hour after a meal, the antacid effect can last up to 3 hours.

Does not cause secondary hypersecretion of gastric juice.

Benzocaine has a local anesthetic effect in the presence of severe pain.

Pharmacokinetics

Algeldrat
Suction- small amounts of the drug are resorbed, which practically do not change the concentration of aluminum salts in the blood.
Distribution- No.
Metabolism- No.
breeding- excreted through the intestines.

Magnesium hydroxide
Suction- Magnesium ions are resorbed in about 10% of the dose taken and do not change the concentration of magnesium ions in the blood.
Distribution- usually locally.
Metabolism- No.
breeding- excreted through the intestines.

Benzocaine
Benzocaine is absorbed in minimal amounts and has virtually no systemic effects on the body. Its local anesthetic effect occurs 1-2 minutes after taking the suspension.

INDICATIONS FOR USE

  • peptic ulcer of the stomach and duodenum (in the acute phase);
  • hiatal hernia, gastroesophageal reflux, reflux esophagitis, duodenogastric reflux;
  • symptomatic ulcers of the gastrointestinal tract of various origins; erosion of the mucous membrane of the upper gastrointestinal tract;
  • acute pancreatitis, exacerbation of chronic pancreatitis;
  • heartburn and pain in the epigastrium after dietary errors, excessive consumption of ethanol, nicotine, coffee, taking drugs that irritate the gastric mucosa.

CONTRAINDICATIONS
Hypersensitivity to any component that is part of the drug. Severe form of renal failure (due to the risk of developing hypermagnesemia and aluminum intoxication).

Do not prescribe to children, as there is a risk of developing methemoglobinemia. Pregnancy and lactation.

Use during pregnancy and lactation
Almagel A is not prescribed during pregnancy and lactation, as it contains benzocaine.

METHOD OF APPLICATION AND DOSES

Adults
5-10 ml (1-2 measuring spoons) 3-4 times a day 10-15 minutes before meals.
The maximum duration of treatment is 7 days, then they switch to treatment with Almagel.
Before use, the suspension must be thoroughly homogenized by shaking the vial!

SIDE EFFECT
Almagel A may cause constipation, which disappears after dose reduction.
In rare cases, nausea, vomiting, stomach cramps, changes in taste sensations, allergic reactions and hypermagnesemia are observed.

With prolonged use of the drug in patients with renal insufficiency and those on dialysis, changes in mood and mental activity are possible. With prolonged use of high doses of the drug, along with a deficiency of phosphorus in food, osteomalacia may occur.

OVERDOSE
With a single dose excess, no other signs of overdose are observed except for constipation, flatulence, and a sensation of a metallic taste.

With prolonged use of high doses, nephrocalcinosis, severe constipation, mild drowsiness, hypermagnesemia are possible. There may also be signs of metabolic alkalosis: changes in mood or mental activity, numbness or muscle pain, irritability and fatigue, slow breathing, unpleasant taste sensations.

It is necessary to immediately take measures to quickly remove the drug from the body by gastric lavage (vomiting induction, activated charcoal intake).

INTERACTION WITH OTHER DRUGS
It can absorb some drugs, thus reducing their absorption, therefore, while taking other drugs, they must be taken 1-2 hours before or after taking Almagel A. Almagel A reduces the acidity of gastric juice, and this may affect the action of a large number drugs while taking.

Almagel A reduces the absorption of reserpine, histamine H2 receptor blockers, cardiac glycosides, iron salts, lithium preparations, quinidine, mexiletine, phenothiazine preparations, tetracycline antibiotics, ciprofloxacin, isoniazid and ketoconazole.

When taking enteric tablets at the same time, an increased pH of gastric juice can lead to an accelerated violation of their membrane and cause irritation of the stomach and duodenum. Almagel A should not be taken simultaneously with sulfonamides, due to the presence of benzocaine in its composition.

Almagel A may affect the results of some laboratory and functional studies and tests: it reduces the level of gastric secretion when determining its acidity; disrupts the diverticulum imaging and technetium bone scintigraphy test (TC99); moderately and for a short time increases the serum level of gastrin, increases the serum level of phosphorus, serum and urine pH.

SPECIAL INSTRUCTIONS
The use of the drug in patients with severe habitual constipation is not recommended; with symptoms of acute appendicitis; in the presence of metabolic alkalosis, cirrhosis of the liver, severe heart failure; in the presence of ulcerative colitis, diverticulosis, colostomy or ileostomy (increased risk of developing disturbances in water and electrolyte balance); chronic diarrhea; acute hemorrhoids; impaired renal function (creatinine clearance During treatment with Almagel A, it is necessary to avoid the use of alcohol and acids (lemon juice, vinegar, etc.), due to the possibility of weakening the local anesthetic effect of benzocaine.

When taking the drug, numbness and anesthesia of the mucous membrane of the oral cavity and tongue occur. This phenomenon is transient and does not require therapeutic measures. The drug does not contain sugar, which allows it to be taken by patients with diabetes. The drug contains sorbitol, which is contraindicated in congenital fructose intolerance.

Influence on the ability to drive a car and work with mechanisms
Almagel A does not affect the ability to drive vehicles and work with mechanisms. When taken in the recommended daily dose, the ethyl alcohol contained in the preparation does not affect the ability to drive a car and work with mechanisms.

Interaction with medicines

When Almagel is taken together with certain medications, their absorption decreases. For this reason, it is advisable to drink medicines an hour before or after drinking the suspension.

Since Almagel lowers the acidity of the gastric secretion, when the suspension is taken in parallel with certain medications, it can affect their effect. For example, when combined with enteric drugs, the high alkalinity of the gastric secretion can prematurely destroy the outer shells of medicines and provoke irritation of the mucous membrane of the digestive organs.

Almagel reduces the impact of:

  • iron salts;
  • lithium preparations;
  • histamine receptor blockers;
  • cardiac glycosides;
  • antibacterial agents of the tetracycline group;
  • phenothiazine drugs.

And also this medicine is able to influence the results of individual diagnostic methods of research.
