Local anesthetic.

pharmachologic effect

A local anesthetic with moderate anesthetic activity and a wide range of therapeutic effects. Being a weak base, it blocks Na+ channels, preventing the generation of impulses in the endings of sensory nerves and the conduction of impulses along nerve fibers. Changes the action potential in the membranes of nerve cells without a pronounced effect on the resting potential. Suppresses the conduction of not only pain, but also impulses of other modalities.

When absorbed and directly vascularly introduced into the bloodstream, it reduces the excitability of peripheral cholinergic systems, reduces the formation and release of acetylcholine from preganglionic endings (has some ganglion-blocking effect), eliminates spasm of smooth muscles, reduces the excitability of the myocardium and motor zones of the cerebral cortex.

Eliminates descending inhibitory influences of the reticular formation of the brain stem. Inhibits polysynaptic reflexes. In large doses, it can cause convulsions. It has a short anesthetic activity (the duration of infiltration anesthesia is 0.5-1 hour).

Pharmacokinetics

Subject to complete systemic absorption. The extent of absorption depends on the site and route of administration (especially vascularization and blood flow rate of administration) and the final dose (amount and concentration). It is quickly hydrolyzed by plasma and liver esterases to form two main pharmacologically active metabolites: diethylaminoethanol (has a moderate vasodilator effect) and para-aminobenzoic acid (is a competitive antagonist of sulfonamide drugs and can weaken their antimicrobial effect). T1/2 - 30-50 s, in the neonatal period - 54-114 s. It is excreted primarily by the kidneys in the form of metabolites; no more than 2% are excreted unchanged.

Indications for use of the drug NOVOCAINE

Infiltration, conduction, epidural and spinal anesthesia, intraosseous anesthesia, anesthesia of the mucous membranes (in ENT practice); vagosympathetic and perinephric blockade. Circular and paravertebral blockades for eczema, neurodermatitis, ischalgia.

Intravenously: to potentiate the action of basic anesthetics; for the relief of pain of various origins.

Intramuscularly: to dissolve penicillin in order to extend its duration of action; as an adjuvant for certain diseases that are more common in old age, incl. endarteritis, atherosclerosis, arterial hypertension, spasms of coronary and cerebral vessels, joint diseases of rheumatic and infectious origin.

Rectal: hemorrhoids, spasms of intestinal smooth muscles, anal fissures.

Dosage regimen

For procaine solution 5 mg/ml (0.5%):

For infiltration anesthesia, 350-600 mg (70-120 ml) is administered. Higher doses for infiltration anesthesia for adults: the first single dose at the beginning of the operation - no more than 0.75 g (150 ml), then during each hour of the operation - no more than 2 g (400 ml) of solution.

For perinephric blockade (according to A.V. Vishnevsky), 50-80 ml is injected into the perinephric tissue.

For circular and paravertebral blockade, 5-10 ml is administered intradermally. For vagosympathetic blockade, 30-40 ml is administered.

To reduce absorption and prolong the effect of local anesthesia, an additional 0.1% solution of epinephrine hydrochloride is administered - 1 drop per 2-5-10 ml of procaine solution.

The maximum dose for use in children over 12 years of age is 15 mg/kg.

For conduction and epidural anesthesia, a 2% solution is used.

Rectally. Having previously freed the suppository from the contour packaging using scissors (cutting the packaging along the contour of the suppository), insert it deep into the anus (after a cleansing enema or spontaneous release of the intestines) 1-2 times a day.

Side effect

From the central and peripheral nervous system: headache, dizziness, drowsiness, weakness, trismus.

From the cardiovascular system: increased or decreased blood pressure, peripheral vasodilation, collapse, bradycardia, arrhythmias, chest pain.

From the hematopoietic organs: methemoglobinemia.

Allergic reactions: skin itching, skin rash, other anaphylactic reactions (including anaphylactic shock), urticaria (on the skin and mucous membranes). If, while using the drug, any of the side effects indicated in the instructions appear or they get worse, or you notice any other side effects not listed in the instructions, inform your doctor.

Contraindications to the use of the drug NOVOCAINE

Hypersensitivity (including to para-aminobenzoic acid and other local anesthetic esters). Children's age up to 12 years.

For anesthesia using the creeping infiltrate method - pronounced fibrous changes in the tissues.

Carefully. Emergency operations accompanied by acute blood loss; conditions accompanied by a decrease in hepatic blood flow (for example, chronic heart failure, liver disease); progression of cardiovascular failure (usually due to the development of heart block and shock); inflammatory diseases or infection of the injection site; pseudocholinesterase deficiency; renal failure; children's age from 12 to 18 years, old age (over 65 years); with caution in seriously ill and/or debilitated patients; during pregnancy and during childbirth.

Use of the drug NOVOCAINE during pregnancy and breastfeeding

During the treatment period, care must be taken when driving vehicles and engaging in other potentially hazardous activities that require increased concentration and speed of psychomotor reactions.

Overdose

Symptoms: pallor of the skin and mucous membranes, dizziness, nausea, vomiting, “cold” sweat, increased breathing, tachycardia, decreased blood pressure, even collapse, apnea, methemoglobinemia. The effect on the central nervous system is manifested by a feeling of fear, hallucinations, convulsions, and motor agitation.

Treatment: maintaining adequate pulmonary ventilation, detoxification and symptomatic therapy.

Drug interactions

Strengthens the inhibitory effect on the central nervous system of drugs for general anesthesia, sleeping pills, sedatives, narcotic analgesics and tranquilizers.

Anticoagulants (ardeparin sodium, dalteparin sodium, danaparoid sodium, enoxaparin sodium, heparin sodium, warfarin) increase the risk of bleeding. When treating the injection site with disinfectant solutions containing heavy metals, the risk of developing a local reaction in the form of pain and swelling increases.

Use with monoamine oxidase inhibitors (furazolidone, procarbazine, selegiline) increases the risk of developing a pronounced decrease in blood pressure. Strengthens and prolongs the effect of muscle relaxants. Vasoconstrictors (epinephrine, methoxamine, phenylephrine) prolong the local anesthetic effect.

Procaine reduces the antimyasthenic effect of drugs, especially when used in high doses, which requires additional correction of the treatment of myasthenia gravis.

Cholinesterase inhibitors (antimyasthenic drugs, cyclophosphamide, demecaria bromide, ecothiopathy iodide, thiotepa) reduce the metabolism of local anesthetic drugs.

The metabolite of procaine (para-aminobenzoic acid) is an antagonist of sulfonamides.

  • Active ingredient: procaine (in the form of hydrochloride) – 2.5, 5, 10, or 20 mg;
  • Auxiliary components: 0.1 M solution of hydrochloric acid - to pH 3.8-4.5; water for injection – up to 1 ml.

The composition of 1 ml of infusion solution includes:

  • Active ingredient: procaine (in the form of hydrochloride) – 2.5 or 5 mg;
  • Auxiliary components: 0.1 M hydrochloric acid solution - up to pH 3.8-4.5, water for injection - up to 1 ml.

The composition of 1 rectal suppository includes:

  • Active ingredient: procaine (in the form of hydrochloride) – 100 mg;
  • Auxiliary component: solid fat - in an amount sufficient to obtain a suppository weighing 1100 mg.

Indications for use

Injection solution

  • Local anesthesia (infiltration, conduction, epidural) during various surgical interventions;
  • Novocaine blockades (perinephric and vagosympathetic);
  • Circulatory and paravertebral blockades for sciatica, neurodermatitis, eczema and other painful conditions;
  • Spasms of peripheral vessels (for relief);
  • Frostbite (to improve microcirculation in the pre-reactive period).

Infusion solution

  • Vagosympathetic cervical, paranephric, paravertebral and circular blockades;
  • Infiltration anesthesia.

Rectal suppositories

Novocaine is prescribed for anal fissures and hemorrhoids (as a local anesthetic).

Contraindications

  • Pronounced fibrous changes in tissues during anesthesia using the creeping infiltrate method (for injection and infusion solutions);
  • Age up to 12 years (for injection and infusion solution) and 18 years (for rectal suppositories);
  • Hypersensitivity to the components of the drug, including hypersensitivity to para-aminobenzoic acid and other local anesthetic esters.

Novocaine should be used with caution in the presence of the following conditions/diseases:

  • Emergency operations, which are accompanied by acute blood loss;
  • Conditions that occur with a decrease in hepatic blood flow (for example, in patients with chronic heart failure, liver diseases);
  • Progression of cardiovascular failure (usually due to heart block and shock);
  • Pseudocholinesterase deficiency;
  • Kidney failure;
  • Inflammatory diseases or infection of the injection site (for injection and infusion solution);
  • Weakened conditions;
  • Proctitis (for rectal suppositories);
  • Pregnancy and childbirth;
  • Breastfeeding period (for injection and infusion solution);
  • Age 12-18 years (for injection and infusion solution) and over 65 years.

Directions for use and dosage

Injection solution

When carrying out infiltration anesthesia, 0.25-0.5% solutions are used, tight creeping infiltration (anesthesia according to the A.V. Vishnevsky method) - 0.125-0.25% solutions. To reduce absorption and increase the duration of action of local anesthesia, a 0.1% solution of adrenaline hydrochloride is usually added to the injection solution (1 drop per 2-5-10 ml of Novocaine solution).

With perinephric blockade (A.V. Vishnevsky method), 50-80 ml of a 0.5% solution or 100-150 ml of a 0.25% Novocaine solution is injected into the perinephric tissue.

When performing paravertebral and circulatory blockades for neurodermatitis, eczema, sciatica, a 0.25-0.5% injection solution is administered.

To improve microcirculation during frostbite, use a mixture containing up to 10-15 ml of a 0.25-0.5% solution. The drug is administered intravenously slowly, preferably in an isotonic sodium chloride solution. The duration of the course is determined by the severity of the pathology (sometimes up to 10-20 injections).

The highest doses are: intramuscular administration - 100 mg (20 ml of 0.5% solution, 5 ml of 2% solution); intravenous administration – 50 mg (20 ml of 0.25% solution).

When performing infiltration anesthesia, the following highest doses are used: at the beginning of the operation (first dose) - up to 1250 mg when using a 0.25% solution (500 ml) and 750 mg when using a 0.5% solution (150 ml). Then, every hour during the operation, no more than 2500 mg is administered when using a 0.25% solution (1000 ml) and 2000 mg when using a 0.5% solution (400 ml).

To relieve spasms, a mixture of 10 ml of a 0.25% solution of Novocaine, 10,000 units of heparin, 2 ml of a 1% solution of nicotinic acid and 2 ml of a 2% solution of papaverine is injected intraarterially slowly.

Infusion solution

For infiltration anesthesia, solutions of 2.5 and 5 mg/ml are used; for tight creeping infiltration (anesthesia according to the Vishnevsky method) - solutions of 1.25 and 2.5 mg/ml. In order to reduce the absorption of Novocaine and prolong its action, a 0.1% solution of epinephrine is additionally administered during local anesthesia (1 drop per 2-5-10 ml of procaine solution).

  • Perinephric blockade (according to A.V. Vishnevsky): 100-150 ml of a solution of 2.5 mg/ml or 50-80 ml of a solution of 5 mg/ml (into the perirenal tissue);
  • Vagosympathetic cervical blockade: 30-100 ml of 2.5 mg/ml solution;
  • Circular or paravertebral blockade for eczema, sciatica and atopic dermatitis: 2.5 mg/ml or 5 mg/ml solution (intradermal).

For infiltration anesthesia for adults, the following highest doses are used: at the beginning of the operation, the first dose is no more than 500 ml of a 2.5 mg/ml solution or 150 ml of a 5 mg/ml solution.

The maximum dose for children 12-18 years old is 15 mg/kg.

Rectal suppositories

Novocaine is used rectally, introducing a suppository deep into the anus after spontaneous bowel movement or a cleansing enema.

Single dose – 1 suppository, frequency of administration – 1-2 times a day.

Duration of treatment – ​​up to 5 days. If there is no improvement, you should consult a doctor.

Side effects

Injection and infusion solution

During therapy, the following disorders may develop: dizziness, headache, weakness, drowsiness, decreased or increased blood pressure, peripheral vasodilation, collapse, arrhythmia, bradycardia, chest pain, tremor, trismus, hearing and vision impairment, persistent anesthesia, nystagmus , methemoglobinemia, hypothermia, allergic reactions (up to anaphylactic shock).

Rectal suppositories

  • Cardiovascular system: arrhythmias, decreased or increased blood pressure, bradycardia, peripheral vasodilation, collapse, chest pain;
  • Central and peripheral nervous system: headache, dizziness, weakness, drowsiness, loss of consciousness, motor restlessness, convulsions, tremor, trismus, nystagmus, visual and hearing impairment, respiratory muscle paralysis, cauda equina syndrome (leg paralysis, paresthesia) , impaired sensory and motor conduction, paralysis of the respiratory center (in most cases with subarachnoid anesthesia);
  • Digestive system: involuntary bowel movements, vomiting, nausea;
  • Urinary system: involuntary urination;
  • Blood system: methemoglobinemia;
  • Allergic reactions: skin rash, itching of the skin, other anaphylactic reactions (including anaphylactic shock), urticaria (on the mucous membranes and skin), dizziness, weakness, low blood pressure (if the described disorders develop, you should stop using Novocaine and consult a doctor);
  • Local reactions: in the first days of therapy, the urge to defecate and a feeling of discomfort may develop, which does not require discontinuation of the drug and subsequently goes away on its own; rarely - itching and hyperemia in the anal area (when using high doses of the drug).

  • Guanadrel, guanethidine, mecamylamine, trimethaphan camsylate (during epidural anesthesia): increased likelihood of developing bradycardia and a sharp decrease in blood pressure;
  • Monoamine oxidase inhibitors (procarbazine, furazolidone, selegiline): increased risk of excessive reduction in blood pressure;
  • Cholinesterase inhibitors (antimyasthenic drugs, cyclophosphamide, demecarium bromide, ecothiopathy iodide, thiotepa): decreased metabolism of procaine;
  • Muscle relaxants: strengthening and prolonging their action;
  • Narcotic analgesics: development of additive effect, increased respiratory depression;
  • Vasoconstrictors (methoxamine, epinephrine, phenylephrine): prolongation of the local anesthetic effect.

Novocaine reduces the antimyasthenic effect of drugs, especially when used in high doses, which requires correction of myasthenia gravis therapy.

Para-aminobenzoic acid (procaine metabolite) is a sulfonamide antagonist.

  3. A 0.5% solution is recommended intradermally.
