High-tech medical care (HTMC) is medical care using high medical technologies to treat complex diseases. Within the framework of VMP, treatment methods using cellular technologies, robotic technology, information technology and genetic engineering methods can be used. All citizens of the Russian Federation have the right to receive free high-tech medical care. The main condition for obtaining it is medical indications.

In what areas of medicine is high-tech care used?

High-tech medical care (HTMC) is medical care using high medical technologies to treat complex diseases. High-tech medical care can be provided in a number of profiles, namely:

• abdominal surgery (treatment of abdominal organs);
• obstetrics and gynecology;
• gastroenterology;
• hematology;
• dermatovenerology;
• combustiology (treatment of severe burn injuries);
• neurosurgery;
• oncology;
• otorhinolaryngology;
• ophthalmology;
• pediatrics;
• rheumatology;
• cardiovascular surgery;
• thoracic surgery (chest surgery);
• traumatology and orthopedics;
• organ and tissue transplantation;
• urology;
• Maxillofacial Surgery;
• endocrinology;
• neonatology;
• pediatric surgery during the neonatal period

How to get High-tech medical care?

Get a referral from your primary care physician

Medical indications for the provision of VMP are determined by the attending physician of the medical organization in which the patient is undergoing diagnosis and treatment. If there are indications, the doctor issues a referral for hospitalization to provide high-tech medical care.

The referral must be made on the letterhead of the sending organization.

Requirements for the direction:

• written legibly by hand or in printed form;
• certified by the personal signature and seal of the attending physician;
• certified by the personal signature of the head of a medical organization (for example, the head physician of a clinic) or an authorized person;
• certified by the seal of the referring medical organization.

The direction must include the following information:

• Full name of the patient, date of birth, address of registration at the place of residence (stay);
• compulsory medical insurance policy number and name of the medical insurance organization (if any);
• number of the insurance certificate of compulsory pension insurance (if available);
• diagnosis code of the underlying disease according to ICD-10;
• profile, group, name of the type of high-tech medical care needed by the patient;
• name of the medical organization to which the patient is sent;
• Full name and position of the attending physician, contact phone number (if available), email address (if available).

Collect a package of documents

A package of documents must be attached to a referral for hospitalization to provide high-tech medical care:

• an extract from the medical documentation, certified by the personal signature of the attending physician, the personal signature of the head (authorized person) of the referring medical organization; the extract must also indicate the diagnosis of the disease (condition), diagnosis code according to ICD-10, information about the patient’s health status, results of laboratory, instrumental and other types of studies confirming the established diagnosis and the need for high-tech medical care;
• a copy of the patient’s passport with a mark of permanent registration in Moscow;
• for children under 14 years of age - a copy of the birth certificate;
• a copy of the compulsory medical insurance policy;
• a copy of SNILS (if available);
• if the patient is a minor - a copy of the passport of the child’s legal representative;
• consent to the processing of personal data of the patient and (or) his legal representative.

Send directions and package of documents

There is a list of types of high-tech medical care that are included in the basic compulsory health insurance program. Your further actions depend on whether the type of medical care the patient needs is included in the compulsory medical insurance list.

From December 6, 2019, a new Procedure for organizing the provision of high-tech medical care using a unified state information system in the field of healthcare, approved by Order of the Ministry of Health of Russia dated October 2, 2019 No. 824n, comes into force (hereinafter referred to as the VMP Procedure, Order No. 824n). From this date, the current Procedure for organizing the provision of high-tech medical care using a specialized information system, which was approved by Order of the Ministry of Health of Russia dated December 29, 2014 No. 930n (hereinafter referred to as Procedure 930n), becomes invalid.

In general, the new VMP Procedure does not differ from Procedure 930n, with the exception of some provisions. Basically, innovations are related to bringing the normative act into compliance with the current legislation of the Russian Federation in the field of healthcare.

And now more about the “new products” of the VMP Order:

A “connection” has been established between high-tech medical care and the unified state information system in the field of healthcare (hereinafter referred to as the Unified State Health Information System). Thus, the organization of the provision of high-tech medical care should be carried out using one of the subsystems of the Uniform State Health Information System, namely the subsystem of maintaining specialized registers of patients for individual nosologies and categories of citizens, monitoring the organization of providing high-tech medical care and sanatorium-resort treatment.

Note that the information of this subsystem of the Unified State Health Information System includes: general information about patients, information about diseases (conditions) of patients, information about prescribed and dispensed medications, information about medical care provided, referral for hospitalization for the provision of VMP, information contained in the Coupon for the provision of VMP, the results of the provision of VMP, recommendations for further observation and or) treatment and medical rehabilitation, others (Section VI of Appendix No. 1).
• . According to paragraph 3 of the VMP Procedure, this assistance is provided IN ACCORDANCE with the procedures for the provision of medical care, BASED ON clinical recommendations and TAKEN INTO ACCOUNT of the standards of medical care. Using the link in Order No. 824n, the regulator clarified that the norm on clinical recommendations begins to apply from 01/01/2022.
• (Clause 7.3 of the VMP Procedure).
• The new procedure establishes the rule that when a patient is referred for the provision of medical treatment, if necessary, consultations (consiliums of doctors) are carried out using telemedicine technologies with remote interaction of medical workers with each other (clause 13 of the Procedure for medical treatment). In general, we can’t live without telemedicine.
• The requirements for the seal of the referring medical organization when making a referral for the provision of medical treatment have been clarified. Thus, this seal must identify the full name of the medical organization in accordance with the constituent documents.
• The regulator decided to do without detailed information about the patient’s identity document, a copy of which must be attached to the referral for hospitalization for the provision of medical treatment. Now medical organizations will have to navigate this issue on their own.
• Refusal for hospitalization will be indicated by an entry in the Voucher for the provision of medical treatment in 3 cases: absence of medical indications for hospitalization of the patient; availability of medical indications for referring the patient for specialized medical care; the presence of medical contraindications for hospitalization of the patient. Currently, the norm of Order 930n provides for a mark in the Coupon only in the case of medical contraindications. We believe that this amendment is purely technical in nature and nothing more.

• And again we return to telemedicine. According to clause 21 of the VMP Procedure, an extract from the protocol of the Commission of a medical organization providing high-tech care (hereinafter referred to as the Commission’s protocol), .... in accordance with the established procedure, is issued to the patient (his legal representative). At the same time, having introduced such a norm, the Ministry of Health made reference to the procedure for organizing and providing medical care using telemedicine technologies (approved by Order of the Ministry of Health of Russia dated November 30, 2017 No. 965n).

  However, it remains a mystery how the Commission’s protocol will be sent to the patient and (or) his legal representative using telemedicine technologies. It is known that a medical organization providing telemedicine services must be linked to the Uniform State Health Information System (either directly or through its own information system). However, this document is not included in the information posted in the Unified State Health Information System (Appendix No. 1 to the RF Government Regulations dated May 5, 2018 No. 555). Moreover, citizens are not users of the Uniform State Health Information System subsystem related to the provision of high medical care.

In accordance with paragraph 3 of the Rules for the formation of a list of federal government institutions providing high-tech medical care not included in the basic compulsory health insurance program, at the expense of budgetary allocations from the federal budget, approved by Decree of the Government of the Russian Federation of November 10, 2012 No. 904 (Collection of Legislation of the Russian Federation, 2012, No. 38, Article 5116; 2015, No. 3, Article 581), I order:

1. Approve federal government institutions that provide high-tech medical care that is not included in the basic compulsory health insurance program, according to the appendix.

2. To recognize as invalid:

Order of the Ministry of Health of the Russian Federation dated October 31, 2016 No. 807n “On amending the annex to the order of the Ministry of Health of the Russian Federation dated December 28, 2015 No. 1014n “On approval of the list of federal government institutions providing high-tech medical care not included in the basic compulsory health insurance program, at the expense of budgetary allocations of the federal budget, the source of which is, among other things, other interbudgetary transfers provided from the budget of the Federal Compulsory Health Insurance Fund to the federal budget" (registered by the Ministry of Justice of the Russian Federation on November 17, 2016, registration No. 44364).

Among them is the Russian Scientific Center for Restorative Traumatology and Orthopedics named after Academician G.A. Ilizarov, Institute of Surgery named after A.V. Vishnevsky, 1472 Naval Clinical Hospital of the Russian Ministry of Defense.

To obtain a referral coupon (quota) to receive high-tech medical care at the FBGU National Medical Research Center for Surgery named after. A.V. Vishnevsky, the patient must contact the regional health authority with the following documents:

• extract from the protocol of the decision of the medical commission
• a written statement from the patient (his legal representative, proxy), containing the following information about the patient:

• An extract from the patient’s medical records must contain information about the state of his health, the examination and treatment performed, recommendations on the need to provide medical treatment, and the results of clinical diagnostic studies on the profile of the disease.
