    Symptoms: A single case of the development of dyspeptic symptoms in the form of diarrhea with blood and pain in the abdominal area was registered when taking the drug orally at a daily dose of 75 g. Apparently, this was associated with the use of a large total dose of sorbitol, which is part of the drug. No other cases of drug overdose have been identified.
    Treatment: In case of overdose, symptomatic therapy is recommended, which may include hemodialysis. There is no specific antidote. The effectiveness of hemodialysis for piracetam is 50-60%.

    Interaction with other drugs
    Piracetam increases the effectiveness of thyroid hormones and antipsychotic drugs (neuroleptics). When administered simultaneously with antipsychotics, piracetam reduces the risk of extrapyramidal disorders. When used simultaneously with drugs that have a stimulating effect on the central nervous system, the stimulating effect on the central nervous system may be enhanced.
    There was no interaction with clonazepam, phenytoin, phenobarbital, or valproic acid.
    High doses (9.6 g/day) of piracetam increase the effectiveness of indirect anticoagulants in patients with venous thrombosis (there was a greater decrease in platelet aggregation, fibrinogen levels, von Willebrand factors, blood and plasma viscosity compared with the use of indirect anticoagulants only).
    The possibility of changing the pharmacodynamics of piracetam under the influence of other drugs is low, since 90% of the drug is excreted unchanged in the urine.
    In vitro piracetam does not inhibit cytochrome P450 isoenzymes, such as CYP1A2, 2B6, 2C8, 2C9, 2C19, 2D6, 2E1 and 4A9/11 at concentrations of 142, 426 and 1422 μg/ml. At a concentration of 1422 mcg/ml, a slight inhibition of CYP2A6 (21%) and 3A4/5 (11%) was noted, however, the Ki level of these two isoenzymes is sufficient when exceeding 1422 mcg/ml, and therefore metabolic interaction with other drugs is unlikely.
    Taking piracetam at a dose of 20 mg/day did not change the peak and curve of the concentration of antiepileptic drugs in the blood serum (carbamazepine, phenytoin, phenobarbital, valproic acid) in patients with epilepsy receiving a constant dosage.
    Co-administration with alcohol did not affect the serum concentration of piracetam, and the serum alcohol concentration did not change with 1.6 g of piracetam.

    Due to the effect of piracetam on platelet aggregation, caution is recommended when prescribing the drug to patients with impaired hemostasis, during major surgical operations, or to patients with symptoms of severe bleeding. When treating patients with cortical myoclonus, abrupt interruption of treatment should be avoided, which may cause resumption of attacks.
    During long-term therapy in elderly patients, regular monitoring of renal function indicators is recommended; if necessary, dose adjustment is carried out depending on the results of a creatinine clearance study.
    Taking into account possible side effects, caution should be exercised when operating machinery or driving a vehicle. Penetrates through the filter membranes of hemodialysis machines.

Piracetam improves cognitive or cognitive processes of the brain, such as memory, learning, attention. Increases mental performance. The drug has an effect on the central nervous system through several mechanisms of action: improves neuronal metabolic processes, changes in the rate of propagation of the excitation process, improves microcirculation due to the stabilization of the rheological properties of blood.
    Improvement of the rheological properties of blood occurs due to the suppression of the aggregation properties of platelets, reducing the adhesiveness of erythrocytes and improving the elastic properties of erythrocyte membranes.
    Does not have a vasodilating effect. The drug also improves cerebral circulation parameters, improves synaptic conduction of neocortical structures and the interaction of the cerebral hemispheres.
    In case of brain dysfunction due to intoxication and hypoxia, nootropil has a restorative and protective effect. There is also a decrease in the duration and severity of vestibular nystagmus.

    Indications for use

    Indications for use of the drug Nootropil are:
    - Coma (including comatose state of post-traumatic and toxic origin), recovery period after coma;
    - For symptomatic treatment of psychoorganic syndrome, including elderly patients with memory loss, mood changes, decreased ability to concentrate, gait disturbance, dizziness, behavioral disorder;
    - Improving mental and motor activity, therapy for emotional and speech disorders in patients who have suffered an ischemic stroke;
    - Alzheimer's disease (including senile dementia of the Alzheimer's variant);
    - Chronic alcoholism - for the treatment of psychoorganic syndrome and abstinence;
    - Psychoorganic syndrome in children with learning disabilities (as a component of complex therapy);
    - Balance disorders, dizziness (except for diseases caused by psychogenic and vasomotor disorders);
    - As part of complex treatment of sickle cell anemia;
    - Cortical myoclonus (as a component of complex therapy or monotherapy).

    Mode of application

    Nootropil prescribed for adults in doses from 30 to 160 mg/kg of patient body weight. The daily dose of the drug is divided into 2-4 times. Nootropil in capsules, tablets and solution for internal use is taken orally before meals or during meals. The liquid for washing down capsules or tablets can be water or juice. If it is impossible to take the drug orally, a parenteral route of administration is prescribed.
    Psychoorganic syndrome: 4.8 g/day for the first 7 days, maintenance dosage - 1.2-2.4 g/day.
    Consequences of stroke (ischemic): 4.8 g/day.
    Comatose states: starting dosage - 9-12 g/day, maintenance dosage - 2.4 g/day. The course of treatment is 3 weeks.
    Balance disorders and dizziness: 2.4-4.8 g/day.
    Sickle cell anemia: prophylactic dose - 160 mg/kg/day (divided into 4 injections). During a disease crisis, it is necessary to increase the dose to 300 mg/kg (intravenously). This dose can also be prescribed in pediatrics - for children from 1 year of age.
    To correct learning disability: 3.3 g/day throughout the entire academic year.
    Chronic alcoholism: during the manifestation of withdrawal syndrome after alcohol withdrawal - 12 g / day, then switch to a maintenance dosage - 2.4 g / day.
    Cortical myoclonus: starting dosage - 7.2 g/day. Every 3-4 days, the dose is increased by 4.8 g/day until the maximum effective dose of 24 g/day is reached. The course of treatment depends on how long the symptoms of the disease continue. However, every 6 months attempts are made to cancel Nootropil or reduce its dose (1.2 g/day every 2 days). Piracetam therapy is discontinued if the drug is insufficiently effective or if there is no therapeutic effect.
    Dose adjustments are made to patients with changes in renal function or the elderly (the latter may experience an age-related decrease in creatinine clearance). Persons with a creatinine clearance of 80 ml/min or more are prescribed the usual dosage. With a clearance of 79-50 ml/min - 2/3 of the recommended dose (in 2-3 doses); 49-30 ml/min - 1/3 dose in 2 doses; 29-20 ml/min - 1/6 of the recommended dose in 1 dose. Nootropil is contraindicated for persons with creatinine clearance less than 20 ml/min.
    No dosage change is required for patients with impaired liver function.

    Side effects

    Nootropil may cause side effects when used:
    From the central nervous system: nervousness (1.13%), hyperkinetic disorders (1.72%), depression (0.83%), drowsiness (0.96%), asthenia (0.23%). They are more common in the elderly when using dosages above 2.4 g/day. The following may also occur: headache, dizziness, ataxia, imbalance, insomnia, exacerbation of epilepsy, agitation, confusion, anxiety, increased libido, hallucinations.
    From the gastrointestinal tract: abdominal pain, vomiting, nausea, diarrhea.
    Allergic reactions: itching, dermatitis, swelling, rash.
    Others: increased body weight (1.29%).

    Contraindications

    :
    Contraindications to the use of the drug Nootropil are: renal failure with creatinine clearance of 20 or less ml/min; hemorrhagic stroke; age up to 1 year; individual hypersensitivity to pyrrolidone, piracetam and other components of Nootropil.

    Pregnancy

    :
    Adequate clinical studies on the safety of use Nootropil Not performed in pregnant women. The active substance of the drug can penetrate into breast milk and through the hematoplacental barrier. In newborns, the concentration of piracetam in the blood is determined to be 70-90% of the concentration in the mother’s blood.
    The drug is not prescribed to pregnant women, except in cases of vital importance of using the drug. If Nootropil is prescribed to nursing mothers, breastfeeding is temporarily abandoned. Experiments conducted on animals have not proven any teratogenic or other effects of piracetam on the developing fetus, pregnancy, childbirth and the newborn.

    Interaction with other drugs

    In combination Nootropil with triiodothyronine, tetraiodothyronine and thyroid extracts, confusion, irritability and sleep disturbances may occur.
    No interaction of the drug with sodium valproate, phenytoin, clonazepam, or phenobarbital has been identified.
    Significant doses of the drug (approximately 9.6 g/day) caused an increase in the effectiveness of acenocoumarol in venous thrombosis, which was manifested by a pronounced decrease in platelet aggregation properties, a decrease in the level of von Willebrand factor, fibrinogen and the viscosity of plasma and blood.
    The likelihood of changes in pharmacodynamic parameters when combined with other drugs is quite low, since 90% of piracetam is eliminated from the body unchanged.
    In vitro studies did not reveal inhibition of the isoenzymes CYP1A2, 2C8, 2B6, 2C9, 4A9/11, 2D6, 2C19, and 2E1 of the cytochrome P450 system under the influence of piracetam at concentrations of 426, 142 and 1422 μg/ml.

    However, the Ki level of both isoenzymes when the dose of 1422 μg/ml is exceeded is quite sufficient. Therefore, metabolic interaction of piracetam with other drugs is unlikely.
    The concentration curve and peak in the blood serum of antiepileptic drugs (carbamazepine, phenobarbital, phenytoin, valproate) did not change when taking piracetam at a dosage of 20 mg/day, provided that the same doses of drugs for the treatment of epilepsy were received.
    The combination of alcohol (ethanol) and piracetam does not change the content of piracetam in the blood serum and the concentration of ethanol (if used simultaneously with 1.6 g of piracetam).

    Overdose

    :
    When the dose of 75 g (oral solution) was exceeded, the symptoms were: abdominal pain, bloody diarrhea (this is probably due to the high concentration of sorbitol in this dosage form of the drug). No specific symptoms of overdose have been identified. In case of overdose Nootropil, taken orally, gastric lavage, initiation of vomiting, symptomatic therapy, hemodialysis are recommended (the effectiveness of the latter is 50-60%). A specific antidote is unknown.
