How to get preferential medicine. How and where to get subsidized medications Availability of medications at regional benefits

Thanks to changes in recommendations and orders of the Ministry of Health Russian Federation, many medications will be available by prescription starting March 1, 2017. The beginning of this year turned out to be stormy for many pharmacy chains, as well as the pharmaceutical industry in general. Some instructions have already entered into force, others will come into force soon. Some orders introduced a moratorium on the work of pharmacists, while others made dispensing prescription drugs significantly more difficult.

What prescription drugs are available as of March 1, 2017?

By order of the Ministry of Health, certain restrictions are introduced on the sale of medicinal products. Implementation medical supplies in one hand is carried out in limited quantities. This refers to the trade in syrups and tinctures that contain ethanol in a concentration of at least 15%. Pharmacists can sell no more than two packages per person. The purpose for which they do this is clear to many. Today, not all drugs are bought by people to fight illnesses. This fact is confirmed by the mass poisoning with hawthorn tinctures in Irkutsk, where more than 70 people were injured.

In addition, pharmacists are prohibited from advising clients on more expensive medications if there are inexpensive analogue. Although, according to many representatives of the pharmaceutical industry, such standards were previously prescribed in the instructions for pharmacy workers.

Pharmacists are also required to provide their clients with information about the characteristics and contraindications of medications, as well as their shelf life, storage conditions and dosage. Previously, such things were not talked about at all.

Prescription drugs

According to the law of the Ministry of Health No. 785, medicines are available by prescription from March 1, 2017. An exception is the list of drugs that can be dispensed without a prescription.

The list of prescription drugs since March 1, 2017 is quite extensive. It includes drugs from literally all medicinal groups. Most of all on the list are antibiotics. According to the ministry's idea, they should all be prescription. Therefore, if we act strictly according to the law, more than 50% of all drugs in pharmacies must be sold only with a prescription from the attending physician.

Opinion of representatives of the pharmacy business

According to representatives of the pharmaceutical industry, at present there are practically no recipes in the classical sense. Doctors write prescriptions on pieces of paper. At the same time, according to the director of one of pharmacy chain Kakhaber Bakuradze, today many pharmacy pharmacists are retraining to new way. He also reports that every day everything more people they come to the pharmacy with prescriptions, and pharmacists work exclusively according to instructions, that is, in accordance with current legislation.

Today sale of prescription drugs medicines without availability necessary documents threatens with a fine of up to 50 thousand rubles. Authorities are considering making penalties for selling prescription drugs without a prescription more severe. It's about about potential cessation of activity for a period of up to 90 days.

Information about over-the-counter medications may be placed on shelves, in wobbler and poster formats, as well as on other media so that the buyer can fully familiarize themselves with the product. In a comfortable view it is necessary to place a price tag with the name, number of doses in the package, dosage, manufacturer.

Barcode on medical products

In addition, another important innovation comes into force - labeling on each medicine. A pilot project of the appropriate type is being launched in several Russian cities. the main objective The measures are to combat counterfeit and counterfeit medical products.

According to preliminary data, this project will help track about six billion packs of medical supplies. At the initial stage in March, barcodes will be placed on 60 product items. Including 10 drugs for the treatment of rare ailments, as well as more than 30 names of drugs that are included in the list of vital necessary medications by prescription since March 2017. It is planned that by the end of this year, automation of the accounting system is considered voluntary. IN next year it will be coercive.

According to Rospotrebnadzor, each package of a medical product at production will be equipped with a two-dimensional barcode. It will be located next to information about the period until which the medicine may be valid. Such a system will allow you to determine the origin of the drug without leaving the pharmacy counter.

Difficulties in using the barcode

However, some problems were also discovered. On mobile device A program must be installed to read the code. There is another option. The buyer can use a pharmacy scanner, which must be installed in pharmacies participating in the project. The system will give the buyer comprehensive information about where, who and when the drug was manufactured.

Kakhaber Bakuradze claims that there should be no problems with the new barcodes. The IT department specialists produced all the required templates long ago. This will make it possible to forever forget about counterfeit and gray drugs, the origin of which is unknown. Honest pharmaceutical businessmen will only benefit from such changes.

Are medicines becoming more expensive?

Many are interested in the question of whether these changes will affect ordinary people in a financial sense? According to pharmaceutical market experts, the changes will not affect the cost of drugs in any way. As they report, the new requirements will be useful to buyers, as well as owners of honest pharmacies that do not use illegal schemes for selling drugs.

The cost of drugs should not increase. This is also evidenced by the prices of drugs in pharmacies in January. Prices have hardly changed.

By the way, in 2017 the film “A Cure for Health” was released, which colorfully illustrates the situation with medicines in our country.

An increasing number of patients are faced with the impossibility of obtaining free medicine prescribed by their attending physician. The reason for this is not only the frequent absence the necessary drugs in pharmacies, but also the dishonesty of their employees who refuse to serve beneficiaries. How to protect your rights?

According to the requirements of Roszdravnadzor, there is a clear algorithm of actions that the pharmacist must adhere to if pharmacy there was no subsidized medicine needed by the patient. But not all citizens know about it. Therefore, having heard a refusal, they purchase expensive drugs at personal funds, while leaving violators of their rights without punishment.

What should a pharmacy employee do if there are no subsidized medications?

If prescribed by a doctor free drugs At the time of the patient's request, the drug is not available in the pharmacy, the pharmacist has the right to offer available similar products. If the client refuses to receive substitute medications, then the pharmacist is obliged to act according to the following algorithm:

  1. Accept a discounted prescription from the patient.
  2. Register it in a special pharmacy journal of unmet demand, assigning it the status of deferred maintenance.
  3. Enter recipe details into electronic program institutions.
  4. Submit a written/electronic request for medications to the supplier company.

The authorized pharmaceutical organization must also register the incoming request and provide an official response to the pharmacy regarding availability/absence and availability this drug. If the application cannot be satisfied on its part, the pharmacy must purchase the drug itself, and the costs incurred will subsequently be compensated by the state.

If the drug is not available in the pharmacy indicated by the doctor who wrote the preferential prescription, the patient has the opportunity to get it at another social pharmacy, provided that it is located on the territory of the same municipality, and the heads of both institutions agreed on this point among themselves. If the drug is not available in the right dosage, the pharmacist can replace it with a medicine with a lower dosage, but increasing its volume to an amount that will be sufficient for therapy. At the same time, to dispense the drug in a larger dosage than prescribed, you must contact your doctor for another prescription. No pharmacy has the right to limit the volume of subsidized medicine. Only the attending doctor has this competence.

How long does it take for the pharmacy to provide medications?

Roszdravnadzor allocates 10 working (not calendar!) days for the delivery of previously unavailable medications. If the drugs were prescribed through a medical commission, then this period increases to 15 days. The client is notified of the arrival of the order often by telephone on the same day that the necessary funds arrive at the pharmacy.

Where should I file a complaint against a pharmacy?

If after specified period delivery of the missing medicine was not carried out, or the pharmacist completely refused service to the client with a preferential prescription, you can first try to resolve the controversial situation by filing a complaint with the manager of the pharmacy. If the violation is not eliminated, you must file an oral or written complaint against the establishment, outlining the essence of the problem. Below are the main ways to file a complaint against a social pharmacy that violates the rights of a beneficiary to receive medicines:

  • Call the hotline of the Department of Health of your city/region. You can find out its number from the help desk operators or on the structure’s website, where you can also familiarize yourself with the specialists’ work schedule;
  • Contact operators « hotline» Pharmacy Department of the Department of Health of your city/region, having found out its contact information in a similar way;
  • Leave a request on the official website of Roszdravnadzor, indicating your contact details, name and address of the pharmacy, detailed description the situation;
  • Contact the administration of the clinic where the preferential prescription was issued. The specialist on duty is engaged in the solution controversial issues problems encountered by patients, including the provision of subsidized medications. Information about his work schedule, as well as a contact phone number, can be obtained from the reception desk;
  • Submit an application to the prosecutor's office against the organization or official who created obstacles to the patient receiving free medicines, attaching copies of the passport, beneficiary ID, and prescription.

Each pharmacy has the right to refuse to issue a medicine to a person if the preferential prescription was not written on an official form or its validity period has expired. In this case, complaints against the pharmacy will not be considered. And the patient will need to contact his doctor with a request for a new prescription.

You need to fill out a prescription for deferred care at the pharmacy. There they are required to register it in the register of unsatisfied demand. Such a magazine should be in every pharmacy that dispenses medications according to preferential prescriptions.

If a prescription is registered in the backlog, it does not need to be left at the pharmacy. The patient takes it with him, but it must be marked with a mark indicating its acceptance for service: date of registration, pharmacy number, signature of the pharmacist.

After registering a prescription, the pharmacy must submit a request for the drug to the pharmaceutical company authorized to supply preferential drugs to the region and provide the drug to the patient within 10-15 days.

The patient can call the pharmaceutical company that supplies subsidized drugs to the region and find out if they have it in stock. necessary drug, when it will be delivered to the pharmacy, for what reason it is missing. You can also notify the company that the prescription has been accepted by the pharmacy for deferred service (the pharmacy may not inform the company about the deferred prescription). The phone number of the pharmaceutical company can be found at the pharmacy.

If 16 days have passed and the drug has not yet arrived at the pharmacy, you need to write a complaint to the authorized pharmaceutical company that you have not received the discounted drug.

You need to wait for a response, which may indicate that the Ministry of Health did not hold scheduled tenders for the purchase of medicines or the medicine was not indicated in the specification for public procurement. The company's response will be the basis for going to court - based on the content of the response, the defendant is determined: the Ministry of Health and Social Development, the pharmacy, pharmaceutical company etc.

Medicine prescribed to a patient free of charge according to a preferential prescription can be purchased for own funds, and then demand compensation from the pharmacy. To do this, you need to write a claim to the pharmacy with a request to reimburse the costs. If the pharmacy does not reimburse the money spent, you can go to court.

The court may refuse to satisfy a claim for compensation Money, if the medicine is purchased independently and the prescription has not been put on deferred maintenance.

The data set allows you to get detailed information about discounted holiday points medicines, get acquainted with their location on the map, as well as find out the exact address, opening hours and other contact information.

The current situation in Russia.

The Strategy for the provision of medicines to the population of the Russian Federation for the period until 2025, approved by the Ministry of Health of Russia in 2013, provides for the pilot implementation of various models of medicine provision.

Co-financing from patients (with the exception of certain categories of citizens for whom free drug coverage is provided) should depend on the nature of the patient’s disease and the cost of treatment, for example:

Costs are fully reimbursed by the state - for life-threatening, socially significant diseases, which make the largest contribution to mortality in Russia,

A high share of reimbursement is provided for drugs for the treatment of non- life-threatening diseases, but requiring long-term expensive treatment,

Low reimbursement rate - for diseases requiring therapy with low-cost drugs.

It is necessary to analyze the results of various models of pilot projects for drug provision with different categories of citizens participating in pilot projects, various diseases(by flow and cost of treatment) to generate calculations of financial costs.

The introduction of drug insurance in the city of Moscow is possible when the Russian Ministry of Health sums up the results of pilot projects in the constituent entities of the Russian Federation within the framework of the Drug Supply Strategy, which are carried out not for individual nosologies, but in full.
