Composition and release form

Teturam tablets are available in 0.15 and 0.25 g in packs of 50 or 30 pieces, as well as powder substance.

  • The main active ingredient of Teturam is disulfiram.

In addition, it contains the following additional components: potato starch, povidone, primellose, colloidal silicon dioxide, stearic acid.

pharmachologic effect

Once in the body, Teturam acts as an inhibitor of altaldehyde dehydrogenase, an enzyme that takes part in the metabolism of ethanol. As a result, the level of acetaldehyde in the blood increases.

This causes symptoms of intoxication:

  • violation heart rate;
  • chills;
  • chest pain;
  • facial redness;
  • nausea with vomiting;
  • general weakness;
  • decreased blood pressure;
  • difficulty breathing.

Due to such symptoms, drinking alcohol ceases to give a person pleasure. Gradually he completely stops drinking alcohol.

After taking the Teturam tablet orally, the active substance is quickly and almost completely absorbed into the systemic bloodstream from the intestine (bioavailability is 70-90%). It is evenly distributed in tissues, metabolized in the liver with the formation of inactive breakdown products, which are excreted from the body mainly in the urine.

Indications for use

The product is used as effective method prevention of chronic alcoholism. Teturam can be used as a drug that guarantees the absence of relapses during the main treatment with other specific methods. The remedy can be used by loved ones both in secret from the alcoholic, and used consciously by a drinker.

The active ingredient Teturam - disulfiram is an antidote to nickel, for this reason the drug can be used for detoxification in case of poisoning with this metal.

Contraindications

Absolute:

Relative (special care is required when using):

  • Peptic ulcer of the stomach and duodenum in remission;
  • Cardiovascular diseases in the compensation stage;
  • Endarteritis;
  • History of psychosis due to taking disulfiram;
  • Residual effects of cerebrovascular accident;
  • Age over 60 years.

Dosage and method of administration

As indicated in the instructions for use, Teturam tablets are taken orally. The drug is prescribed after a full medical examination; the doctor must warn the patient about possible consequences and complications.

  • Recommended dosage: according to an individual regimen from 150 to 500 mg 2 times a day. Teturam-alcohol test is carried out after 7-10 days of therapy; it consists of the patient taking 20-30 ml of vodka (40%) after 500 mg of the drug. In case of a weak reaction, the amount of vodka is increased by 10–20 ml. The dose of alcohol should not exceed 120 mg.

Repeated tests with adjustment of doses of alcohol and/or drug are carried out after 1–2 days in the hospital and after 3–5 days on an outpatient basis. Maintenance therapy in daily dose 150–200 mg can be continued for 1–3 years.

Tetra-alcohol tests must be carried out under the supervision of a physician, since it is impossible to predict the reaction to alcohol in each specific case. Often a person experiences severe, poorly tolerated sensations that require quick fix. To relieve painful and excessive reactions during tetra-alcohol tests, a number of medications are used, most of which are administered intravenously or intramuscularly. It is precisely because of the likelihood of such an intervention that the doctor must be present and monitor the progress of the alcohol test.

special instructions

Use with caution in patients with renal failure, diseases of the liver, respiratory system.

Interaction with other drugs

When using the drug, it is necessary to take into account interactions with other medications:

  1. When used simultaneously with isoniazid, cases of dizziness and depression have been described; with caffeine - the excretion of caffeine from the body decreases; with metronidazole - develops acute psychosis, confusion; with omeprazole - a case of the development of impaired consciousness and catatonia is described.
  2. When used concomitantly with perphenazine, the development of psychotic symptoms cannot be excluded.
  3. When used concomitantly, disulfiram inhibits the metabolism and excretion of rifampicin.
  4. When used simultaneously with coumarin derivative anticoagulants (including warfarin), the anticoagulant effect is enhanced and the risk of bleeding increases.
  5. Disulfiram inhibits liver enzymes, therefore, with simultaneous use of drugs metabolized in the liver, their metabolism may be disrupted.
  6. With simultaneous use of disulfiram with tricyclic antidepressants, phenothiazine derivatives, MAO inhibitors, there is a risk of developing serious adverse reactions associated with drug interactions.
  7. A case of the development of mania in a patient taking disulfiram and buspirone is described.
  8. With simultaneous use, the clearance of desipramine and imipramine from the body decreases.
  9. When used simultaneously with amitriptyline, it is possible to enhance the therapeutic effect of disulfiram, but it is also possible to enhance toxic effect amitriptyline on the central nervous system.
  10. With simultaneous use, the plasma concentration of diazepam and chlordiazepoxide increases, in some cases accompanied by dizziness. Under the influence of diazepam, the intensity of the disulfiram-ethanol reaction may decrease. A case of increased toxicity of temazepam has been described.
  11. When used simultaneously with phenazone, T1/2 of phenazone increases; with phenytoin - the effects of phenytoin are enhanced due to an increase in its concentration in the blood plasma, toxic reactions develop.
  12. When used simultaneously with chlorzoxazone, the concentration of chlorzoxazone in the blood plasma increases; with chlorpromazine - possible enhancement arterial hypotension.

To develop an aversion to alcohol, doctors use the drug Teturam, which, when interacting with even a small amount of alcohol, causes convulsions, dilates blood vessels and leads to nausea. The use of the drug is justified in cases of treatment of alcoholism. The principle of operation of the drug is that it inhibits the enzymes acetaldehyde dehydrogenase involved in the metabolism of ethanol.

Instructions for use Teturama

The drug based on the substance disulfiram enjoys well-deserved popularity and is capable of bringing real benefit subject to compliance with the rules of use. The action of the medication is based on the principle of “knocks out a wedge with a wedge,” so it must be used with caution, following the instructions. You need to realize that Teturam promotes short-term severe intoxication of the body, which creates a conditioned reflex in the drinker: alcohol means pain and suffering. The drug can influence the addict’s behavior much more strongly than the persuasion of loved ones.

Composition and release form

The drug is available in the form of tablets with a flat-cylindrical shape. The color of the medicine is white, sometimes with a yellow-greenish tint, and there is no odor. The drug is intended for oral administration. The tablets are packaged in blisters, which are placed in carton boxes for sale through pharmaceutical networks. The volume of the active substance can be 100, 150, 200 mg.

Pharmacodynamics and pharmacokinetics

The drug affects the metabolism of the body ethyl alcohol, as a result of which the concentration of toxic breakdown products of alcohol increases in tissues. The person begins to feel a rush of blood to the face, a urge to nausea ( possible complications, reaching the point of vomiting), changes blood pressure. The peak severity of the therapeutic effect is achieved 12 hours after administration and can last up to two weeks.

The absorption of the drug in the stomach and intestines is at least 70% and can reach up to 90%. During the metabolic process that occurs in short time Teturam is reduced to dithiocarbamate, which is excreted from the body as a glucuron conjugate. Part of the dithiocarbamate can be transformed into diethylamine and carbon sulfite, which can be excreted through the lungs, the remaining parts are excreted in the urine.

Indications for use

The drug is used as an effective method of preventing chronic alcoholism.

Directions for use and dosage

According to the instructions, Teturam is taken orally orally. Dosages should be in the range of 150-500 mg per day. The specific dosage and regimen are determined individually by a narcologist depending on the patient’s condition. After 8-10 days, an experimental test of the effect of the product is carried out. The test produces a psychological effect. After taking the tablet, the patient is given 30 mg of vodka. The strength of the reaction is a factor in dosage adjustment. After the revision, a second test is carried out after 3-4 days. The maintenance dose is 150-200 mg per day.

How to take Teturam on your own

Often the drug is used by patients independently or through loved ones. This is an acceptable practice, but it must be preceded by a mandatory consultation with a doctor with a prescription for a dosage regimen. The doctor has knowledge about the risks of using the drug against the background of the patient’s pathologies and health conditions. Neglecting these factors can cause harm. According to doctors, 1-3 tablets per day (150-450 mg) are prescribed. The admission process should be accompanied by periodic monitoring by a doctor.

Duration of treatment

The duration of the main course of therapy is 4-6 weeks. A maintenance dose can last from one to three years. The decision on specific dates must be made by the responsible doctor. The decisive influence on the treatment period is exerted by individual characteristics sick. The duration and reliability of therapy largely depend on the complexity of the approach. Measures for social adaptation the patient consolidates the effect achieved with Teturam. It is possible to implant tablets.

Teturam and alcohol

The therapeutic effect is achieved by suppressing the drug’s ability of the liver to produce acetaldehyde dehydrogenase, an enzyme that breaks down alcohol into water and carbon dioxide stopped at the intermediate stage of acetaldehyde. This substance is toxic poison, which according to negative impact 20-30 times stronger than alcohol. The concentration of acetaldehyde accumulates in tissues and causes a number of manifestations in a person addicted to alcohol, as a result of which he is forced to stop the drinking process alcoholic drinks.

Drug interactions

Teturam tablets cannot be combined with certain medications due to negative reactions. Example of combinations and effects:

  • According to reviews, anticoagulants, coumarin derivatives, Warfarin increase the risk of bleeding;
  • disulfiram disrupts the metabolism of liver enzyme inhibitor drugs;
  • tricyclic antidepressants, phenothiazine derivatives increase the risk of developing side effects;
  • Buspirone may cause mania when combined with Teturam;
  • the drug reduces the clearance of Desipramine, Imipramine;
  • Amitriptyline enhances the therapeutic effect of the drug, but at the same time the toxicity of the drug itself increases;
  • Disulfiram increases the plasma concentration of Diazepam and Chlordiazepoxide, their use is accompanied by dizziness, Diazepam in addition reduces the intensity of the reaction with ethanol, there are cases of increased toxicity of Temazepam;
  • Isoniazid can cause dizziness, depression, caffeine - decreased excretion from the body, Metronidazole - acute psychosis and confusion, Omeprazole - impaired consciousness and catatonia;
  • Perphenazine can lead to psychotic symptoms;
  • Disulfiram inhibits the metabolism and excretion of Rifampicin, increases the withdrawal period of Phenazone, enhances the effect of Phenytoin, increases the concentration of Chlorzoxazone, and enhances the effect of arterial hypotension of Chlorpromazine.

Side effects of Teturama

The most common side effect of the drug is a teturam-like effect (total alcohol intolerance due to the biotransformation of alcohol), which cannot be considered an achievement, since alcohol is produced in minimal doses human body default. Others negative reactions from taking medication are:

Video

A drug for the treatment of alcohol addiction. It has an inhibitory effect on the enzyme aldehyde dehydrogenase, which is involved in the metabolism of ethanol. This leads to an increase in the concentration of the ethanol metabolite - acetaldehyde, which causes flushing, nausea, vomiting, general malaise, tachycardia, and a decrease in blood pressure, which make drinking alcohol extremely unpleasant after taking disulfiram.

Pharmacokinetics

After oral administration, rapid but incomplete (70-90%) absorption occurs active substance from the gastrointestinal tract.

Due to its high solubility in lipids, disulfiram is widely distributed in the body and accumulates in various fat depots.

Disulfiram is rapidly metabolized to diethyldithiocarbamate (DDC), which is partially excreted as carbon disulfide in exhaled breath and partially metabolized in the liver to methyl-DDC. The latter is converted into an active metabolite - diethylthiocarbamic acid methyl ester (methyl-DTC). Cmax in blood plasma of methyl-DTC is achieved 4 hours after taking disulfiram, but the maximum inhibitory activity against aldehyde dehydrogenase is first observed after 3 days of administration.

T1/2 methyl-DTC is about 10 hours, while the inhibitory activity against aldehyde dehydrogenase lasts much longer. The effect may therefore persist for 7-14 days after discontinuation.

With a slight or moderate deterioration in liver function, metabolism does not change. On the contrary, with liver cirrhosis, an increase in the concentration of metabolites in the blood is observed.

Metabolites are excreted mainly in the urine. Some is excreted in exhaled air in the form of carbon disulfide. 20% is excreted through the intestines as unchanged disulfiram.

Up to 20% of the dose may remain in the body for 1 week or more.

After implantation, disulfiram penetrates into the blood. The minimum content of disulfiram in the blood is about 20 ng/ml. The content of DDC and carbon disulfide in the blood reaches 0.14 μg/ml after implantation of 1-1.6 g of disulfiram. However, despite these very low levels, disulfiram-ethanol reactions can occur within 5-9 months after implantation.

Teturam – reliable and effective medicine, which is used to treat alcoholism. Usually this remedy is prescribed when other drugs do not have the desired effect. An overdose often leads to neurological complications, depression of consciousness, and coma, so it is extremely important to know how to promptly and quickly remove Teturam from the body.

General information about the drug

The medicine contains a substance called disulfiram. In addition to this active component, there are additional auxiliary components. The manufacturer produces the drug in tablets and powder.

The main effect of the drug is the blockade of acetaldehyde dehyrogenase. This component increases the content of acetaldegin and affects the biotransformation of ethanol. As a result, when a person drinks alcoholic beverages, the biotransformation of ethanol does not occur, and alcohol accumulates in the body at the level of acetaldehyde (see). Then the person’s face turns red, sometimes vomiting occurs, the heart rate increases, malaise occurs, blood pressure decreases, and a feeling of fear appears.

Taking Teturam provokes all the effects described above, as a result of which a person develops a conditioned reflex in which an aversion to alcohol occurs. It is precisely this aversion from alcohol and the presence side effects counts therapeutic effect medicines.

In medicine, the drug is sometimes stitched under the skin. They also take tablets and powder to dissolve in water.

Interesting!The therapeutic effect develops within four hours, but after discontinuation of the drug, the effect of the drug continues for two days.

When the tablets are implanted under the patient’s skin, the medicine begins to act immediately, and after their removal, the effect is observed for another 5–9 months. The drug is excreted in the urine.

Indications for use Teturam

This remedy is indicated for patients for the treatment of alcoholism. Taking the medicine provokes an aversion to alcohol in patients. But you can take this remedy only when other drugs do not help, since the use of Teturam often causes severe side effects.

Teturam is shown:

  • for chronic drunkenness, as well as for the prevention of binges and breakdowns
  • as a treatment for nickel tetracarbonyl poisoning;

The subcutaneous tablet is indicated for use in the treatment of chronic drunkenness, and also as an anti-nickel agent.

How is the medication taken?

The use of Teturam tablets is carried out in three stages:

  1. The doctor examines the patient and prescribes a dose of the drug to active ingredient accumulated in the blood in a fairly high concentration.
  2. Carrying out alcohol tests. Their main principle is to drink alcohol immediately after using a high (1.5–2 times the usual) dose of the drug and causing side effects from the drug. Such tests are carried out only under the supervision of a doctor. Their main task is to form conditioned reflex by aversion to alcohol.
  3. After tetra-alcohol tests, low doses of the drug are continued for 3 years.

The daily dosage of Teturam is 150–500 mg 2 times. The sample period is 7–10 days. Maintenance therapy – 1–2 days in a hospital setting, 3–5 days on an outpatient basis. 1–3 years of maintenance therapy.

Teturama overdose

Clinical manifestations Teturam overdoses manifest themselves in the form of neurological complications and cardiovascular collapse. A depressed state is also observed. Teturam poisoning is usually divided into three forms:

  1. Spicy.
  2. Chronic.
  3. Alcohol turam reaction.

The acute form of drug poisoning occurs 6–12 hours after the tablet is taken. In this case, ataxia, tachycardia, the smell of acetone or sulfur from the mouth, and blepharospasm are observed. The person may fall into a coma.

The chronic stage is characterized by weakness, the appearance of dermatitis, neuropathy and appears 6–12 months after taking the medicine.

Alcoholteturam reactions occur 5–10 minutes after the patient taking Teturam drinks alcohol. At the same time, the skin of the face turns red, dizziness, fear occurs, vomiting and nausea appear, pain in the chest, hypotension.

An overdose of Teturam can occur not only after drinking alcohol, but also after taking wine vinegar or alcohol-containing medications, for example, Nitroglycerin, Valocordin, and some cough elixirs.

Important! A life-threatening dose is a blood ethanol level of 50 mg or higher. This is 0.3–0.6 g/kg of human weight. An overdose in this case develops immediately, within 5–10 minutes after drinking alcohol. The reaction lasts approximately 2–3 hours. If measures were taken on time, a favorable outcome occurs within 4 hours.

The clinical picture of severe drug poisoning is characterized by headaches, nausea and repeated vomiting. The person does not realize what is happening to him, his blood pressure drops sharply, and loss of consciousness occurs.

In order to provide timely and correct assistance to the patient, he needs to know and tell the doctor that he is taking Teturam. Unfortunately, sometimes there are situations when the patient does not know that he is “on pills,” while relatives secretly slip drugs into his food. In such situations, it is extremely difficult to diagnose the cause of all the symptoms described above. Confirmation that poisoning occurred while taking Teturam is a blood test for ethanol.

How to remove Teturam from the body

Antidote for rapid removal the substance does not exist from the body. That's why doctors start symptomatic treatment, which helps reduce the severity of drug overdose:

  1. Coma patients are intubated. If they experience hypotension, use infusion therapy and administration of Norepinephrine.
  2. At the second stage of treatment, Pyridoxine is used in a dose of up to 1 g. If vomiting occurs, it is eliminated using Reglan or Ondansetron. Hyperthermia treated with use of NSAIDs(non-steroidal anti-inflammatory drugs).
  3. At the third stage they use high doses vitamin C up to 1 g. The substance is administered as an effective antioxidant. Summarizing

Unfortunately, it is impossible to quickly remove the drug from the body; the effect will be observed for another two weeks after stopping the medication. The fact is that the active component accumulates in the subcutaneous fat layer, from there it enters the blood in small doses. Due to this, the concentration of the component in the blood is constantly maintained and is eliminated only two weeks after the drug is completely discontinued.

For quick "decoding" that is not accompanied by severe poisoning, the patient is usually given conditional Teturam antidotes, which only partially eliminate unpleasant symptoms caused by the drug. These drugs include: 1% solution of methylene blue, Suprastin, Diazolin, Erius, Parlazin. Unfortunately, it is impossible to remove the medicine from the body earlier.
