Purpose of the study. Evaluation of the comparative effectiveness of the use of Remens and low-dose hormone replacement therapy in the treatment of patients with menopausal syndrome (CS).

Material and methods. 180 patients with various manifestations of CS were under observation. All underwent a comprehensive clinical and laboratory examination, assessment of psycho-emotional and vegetative status before treatment with Remens and Femoston 1/10, 1/5, as well as after 6 and 12 months.

Research results. After the complex treatment, after 12 months in all groups of patients, all the main symptoms of CS either practically disappeared, or they occurred in a mild form. Positive changes in psycho-emotional and vegetative status were obtained in all examined patients.

Conclusion. The results obtained by us in the course of the study allow us to consider the prescription of the complex homeopathic remedy Remens justified, as effective method, in the treatment of menopausal syndrome, including in combination with low-dose HRT preparations.

According to WHO (1996) in the world in 1990, the number of menopausal women was 457 million, and by 2030 it will increase to 1 billion 200 million people. More than 21 million women over 50 live in Russia, 8.5 million of them suffer from age-related problems. 80% of women during perimenopause note certain manifestations of menopausal disorders, and 50% of them need long-term medical correction. The progressive increase in women's life expectancy, the high incidence of menopausal disorders, and the decline in the quality of life cause an increase in socio-economic and medical problems.

There is no doubt that the main method of correcting menopausal disorders is hormone replacement therapy (HRT) with various analogues of sex steroid hormones. The frequency and type of HRT used vary widely (10-40%), which is due to both socio-economic factors and the preparedness of the medical community and the population as a whole. Number of women taking hormonal agents in perimenopause in Russia, is 1-4%. Still not overcome negative attitude patients to the use of hormonal drugs in the relief of menopausal disorders. So, according to statistical studies, in 20% of cases, women stop hormone therapy after 9 months, and about 1/3 do not start taking after a doctor's prescription. The results of studies by domestic authors showed that patients continue HRT for more than 3.5 years only in 61% of cases. Among women who discontinued therapy, refusal to take in the first year was established in 40% of cases, after 1-2 years - in 49%, after 3 years - in 11%.

This circumstance is to a certain extent due to the presence of absolute and relative contraindications, as well as intolerance to drugs and the development of a number of side effects against the background of long-term use of HRT (thromboembolism, heart attacks, breast cancer, etc.).

In connection with the foregoing, the search for alternative methods of treating menopausal disorders is of great interest, where the leading role undoubtedly belongs to homeopathic medicines. natural origin used in small doses. Of particular note is the use of complex homeopathic remedies, such as Remens (Richard Bittner AG). The main idea of ​​prescribing homeopathic remedies is to balance the state of the body using constitutional therapy (the principle of "Law of Similarity") and the creation of therapeutic systems based on the symptoms of the disease. A number of scientific publications convincingly prove the effectiveness of prescribing Remens for the treatment of dysmenorrhea, secondary amenorrhea, premenstrual syndrome, menopausal syndrome and inflammatory diseases of the pelvic organs as part of complex anti-inflammatory therapy.

At the same time, in the literature available to us, we did not find reports on the use of Remens in comparison with the already proven and officially recognized hormone replacement therapy for patients suffering from menopausal syndrome.

aim The purpose of this study was to evaluate the comparative effectiveness of Remens and low-dose hormone replacement therapy in the treatment of patients with menopausal syndrome.

Design and research methods: conducted a non-randomized, non-placebo-controlled, prospective, comparative, clinical study in 180 women aged 45-58 years with various clinical manifestations climacteric syndrome. The examination included generally accepted clinical, laboratory, instrumental, ultrasound, radiological and endoscopic methods research, determination of the level of FSH, LH, prolactin, estradiol, progesterone and cortisol. In addition, the severity of the climacteric syndrome was assessed using the modified menopausal index (MMI), proposed by E.V. Uvarova in 1983, the determination of the vaginal health index (VHI) and densitometry to detect mineral density bone tissue(BMD) with the definition standard deviation(SD) by T-test.

To assess psychosomatic status, we used following tests: the Spielberger self-esteem scale modified by Yu. Khanin to determine the state of reactive and personal anxiety; MFI scale - 20 for subjective assessment of asthenia; visual analog scale (VAS) for quality of life.

The inclusion criteria were women over 45 years of age with a diagnosis of climacteric syndrome established as a result of the examination with a disease duration of more than 1–8 years. Exclusion criteria: signs of obvious mental or hysterical disorders, severe extragenital somatic diseases, fibroids, endometriosis, recurrent endometrial hyperplastic processes, as well as generally accepted contraindications to the use of substitution hormone therapy.

All examined women were divided into 3 equal groups of 60 patients each. The groups were selected according to the "case - control" principle and were identical in age, region of residence, profession and the presence of concomitant pathology. 60 patients of the first group took the homeopathic preparation Remens 1 tab. / 10 drops 3 times / day for 12 months. The homeopathic preparation Remens is a complexone, which includes Cimicifuga (Cimicifuga D1), Scandinavian Sanguinaria (Sanguinaria D6), Pilocarpus (Jaborandi D6), Cuttlefish gland secret (Sepia D12) and Surukuku snake venom (Lachesis D12). 60 women of the second group received CS therapy, which included taking Femoston in two forms, depending on the phase of menopause 1/10 or 1/5, with a minimum content of estrogen (1 mg) and dydrogesterone (5-10 mg). 60 patients of the 3rd group with more severe clinical symptoms of KS were treated simultaneously with the homeopathic preparation Remens and the hormonal preparation Femoston 1/10 or 1/5. The duration of the course of treatment was also 12 months.

Research results and discussion

An assessment of the severity of the climacteric syndrome in patients of the 1st group examined by us states the predominance of early symptoms menopausal disorders such as hot flashes, excessive sweating, headaches, increased blood pressure, palpitations, irritability, anxiety, weakness, drowsiness, etc. In addition, in 1/3 of postmenopausal women, pronounced atrophic processes in the vagina (IVR = 2 points) and osteopenia (SD value according to T-criterion = -2.5) were detected. The overall assessment of the severity of the syndrome was 52.2 points (Table 1), which corresponds to moderate degree severity of menopausal disorders according to MMI.

Special attention we paid attention to the presence of neuropsychiatric, emotional and vegetative disorders in patients. It was found that in all 60 patients with different frequency emotional, cognitive, vegetative, motivational, asthenic and endocrine-metabolic symptoms are diagnosed (Table 1). The level of asthenia according to the MFI scale - 20 was the maximum in all patients (20 points). To assess the level of reactive (RT) and personal (LT) anxiety, we tested women in the main and control groups according to the Spielberger-Khanin scale. Total average level RT and LT in this group of examined women amounted to 70.1 points.

Analyzing the effectiveness of the use of the homeopathic remedy Remens in the treatment of patients suffering from CS, it is necessary to emphasize it highly statistically. significant influence on the normalization of psycho-vegetative, endocrine-metabolic, emotional and motivational clinical symptoms disease, which is clearly demonstrated by the data shown in Table 1. Already after 6 months from the start of the therapy, a decrease in the severity of the menopausal syndrome according to the MMI data, normalization of the state of the vaginal epithelium, a decrease in osteopenia, a decrease in the level of FSH from 49.8 to 27.8 IU / l, an increase in the level of estradiol in the blood from 110.2 to 201.7 pmol / l. Moreover, after 12 months of prospective observation of patients, the above positive trends increased, the symptoms of asthenia leveled out, and the picture of personal and reactive anxiety normalized. According to the VAS quality of life data, a pronounced positive effect was noted by 45% of women, a significant improvement - by 50%, only 3 patients with a severe CS picture rated their quality of life as "no change" (Table 4). No side effects have been identified during treatment with Remens, i.e. the drug has good compliance.

Our observations confirm the versatility of pathogenetic mechanisms therapeutic effect Remensa on pathological processes, forming clinical picture climacteric syndrome. First of all, this is due to the potentiation of the therapeutic effect of all components of this complex homeopathic preparation. After the publication by the father of homeopathy S. Hahnemann of the "Organon of Medical Art" in 1810, an in-depth study of the therapeutic effects of homeopathic remedies began, with the allocation of constitutional remedies of a predominantly female orientation.

So, it is well known that Cimicifuga, which is part of Remance, removes everything well functional disorders associated with improper functioning of the ovaries, especially in women with a labile unstable psyche, hysteroid manifestations and asthenia phenomena. From the point of view of modern concepts, phytoestrogens were found in the composition of Cimicifuga, the drug has an estrogen-like effect, selectively reduces the concentration of FSH in the blood and is a phytoselective estrogen receptor modulator (phytoSERM). Cimicifuga effectively eliminates a complex of vegetative disorders of the menopause (hot flashes, sweating, etc.), prevents the development cardiovascular pathology and postmenopausal osteoporosis. There is an assumption that many alkaloids isolated from Sanguinaria affect the central and vegetative nervous system by inhibition of acetylcholinesterase.

Pilocarpus also effectively eliminates excessive sweating, hot flashes, nausea, nervousness, normalizes thyroid function. The main biological components of Lachesis are peptides and enzymes that effectively stop the main clinical symptoms of menopausal disorders in both mild and severe cases. It has been proven that Sepia normalizes the function of the hypothalamic-pituitary-ovarian system, also eliminates excessive sweating, headaches, depression, apathy, irritability, indifference. Our studies have shown that the main, most frequent vasomotor symptom complex of perimenopause is specific vegetative-vascular paroxysms - the so-called "tides". The pathophysiology of hot flashes has not yet been clarified. In a literature review on this issue, O.A. Gromova et al. (2010) identifies three approaches to the pathophysiological explanation of hot flashes: vegetative, withdrawal, and hormonal-neurotransmitter. From the position evidence-based medicine substantiates the effectiveness of non-hormonal methods of relief of hot flashes, in particular by prescribing the drug Klimalanin (beta-alanine). From our point of view, the optimally selected composition of all the ingredients of the Remens homeopathic complexon also effectively stops the main vegetative-vascular paroxysms both at the peripheral and central levels, which ultimately leads to stabilization of the functioning of the hypothalamus - the most complex interface between the nervous and endocrine system, including the functions of the thermoregulatory center located in it.

The issue of the use of HRT drugs in the early postoperative period to prevent the development of surgical menopause due to possible thromboembolic complications, as well as the appointment of HRT drugs as an "add-back" therapy in connection with leveling is debatable. healing effects gonadoliberin agonists. We have summarized our own clinical experience successful relief of menopausal disorders in 6 patients in the early postoperative period after total hysterectomy, as well as in 8 patients treated with Buserelin, using Remens without negative influence on the hemostasis system, without activation of endometriosis and without a tendency to the growth of myomatous nodes.

Analyzing the clinical effectiveness of the relief of menopausal syndrome using hormone replacement therapy with Femoston 1/10 or 1/5, we confirmed the data obtained by other authors. So, after 12 months, while taking the drug, MMI decreased from 53.1 to 16.2 points, the IVZ showed normal condition vaginal mucosa, osteopenia disappeared, FSH level decreased from 52.2 to 18.1 IU/l, blood estradiol level increased from 108.5 to 440 pmol/l, blood pressure returned to normal, palpitations disappeared, asthenia score, personal and reactive anxiety approached normal values. All data obtained were statistically significant (Table 2). According to VAS quality of life expressed therapeutic effect noted 35.6% of the surveyed women after 12 months of taking HRT, improvement - 59.3%, 5.1% of women rated their condition "no change" (Table 4). Our data confirm the feasibility of using a low-dose drug for HRT, which is Femoston 1/10 and 1/5, since it contains minimal effective dose 17-beta-estradiol (1 mg) and a metabolically neutral progestogen - dydrogesterone, which minimizes the possible side effects of HRT.

With regard to the comparative effectiveness of the combined use of the homeopathic remedy Remens and low-dose hormonal drug Femoston, the statistically significant data presented in Table 3 convincingly demonstrate the cumulative effect of these two drugs on the relief of all clinical symptoms of menopausal syndrome, especially in severe cases. We would like to emphasize that, unlike the two previous observation groups, all patients after 12 months of therapy noted a pronounced therapeutic effect or a significant improvement in overall well-being and quality of life (Table 4). This circumstance fits into the theoretical prerequisites for the possibility of developing an abstinence syndrome in the pathogenesis of specific vegetative-vascular paroxysms, which dictates the need for a balanced approach to prescribing HRT drugs, the predominant use of low-dose drugs and the refusal to abruptly cancel HRT with the transfer of patients to homeopathic methods of treatment in order to prevent the recurrence of symptoms. .

Conclusions:
